America Heart Association (AHA) established the concept of “Ideal Cardiovascular Health” in 2010. Its relation to the prevalence of atrial fibrillation is still uncertain. Our study investigated the association of cardiovascular health behaviors (CVH) with prevalence of atrial fibrillation (AF) in a community-based population.
A total of 4475 participants (selected from Jidong community) aged 40 years or older, were enrolled in medical center of Jidong Oilfield Inc from 2013 to 2014. We collected specific information about the 7 CVH metrics: smoking, body mass index (BMI), diet, physical activity (PA), total cholesterol (TC) and fasting blood glucose (FBG). Assessment of atrial fibrillation (AF) performed by standard 12-lead electrocardiogram (ECG) or medical history. A multivariable logistic regression model was used to estimate relationship between the ideal CVH and prevalence of AF.
Of the 4477 individuals, 48 (1.1%) AF was reported. Participants with a higher number of ideal components had a reduced likelihood of having AF compared with those has few number of ideal components (95%CI: 0.62-0.97, p-trend=0.024) in persons above 40 years old, especially in men and middle-aged (40-60 years old) population. Diet, physical activity and FBG show the significance in this relationship. Participants meeting 3-4 or 5-7 ideal CVH components had 59% (95CI%: 0.22-0.80) and 56% (%95CI: 0.20-0.98) decrease the odds of AF. Both the ideal behaviors and factors were associated with lower prevalence of AF in participants with more ideal behaviors/factors, compared with those meeting 0-1(95%CI: 0.44-0.96/0.68-0.93, p=0.03/0.004, respectively).
Based on community population, our study suggested that the numbers of ideal cardiovascular components significantly decrease the prevalence of AF. From a public health perspective, it is necessary to redouble our primordial prevention efforts at individual level.
