QRS complex duration changes due to acute myocardial infarction and after Percutaneous Coronary Intervention especially with no-reflow phenomenon have been described in many studies previously, however their relation with tissue reperfusion condition after primary PCI are not evident yet.This study aimed to investigate the value of QRS duration as indicator of tissue reperfusion in patients with acute ST-segment elevation myocardial infarction after successful primary PCI compared to myocardial blush grade.
The study included forty three patients presented by STEMI and managed by primary PCI, they divided according to myocardial blush grade into two groups, good reperfusion group A with MBG 2&3 (included twenty six patients, 22 males, 4 females, with average age 57.58±9.57 years) and poor reperfusion group B with MBG 0&1 (included seventeen patients, 14 males, 3 females, with average age 62.35±9.29 years).
Although the two groups did not show significant difference in QRS duration on admission (mean value in group A 87.46±11.7 vs. 88.65±13.02 msec in group B, p=0.757), there was a significant difference in QRS duration after primary PCI (in group A mean duration 79.08±7.57 msec with mean duration narrowing 8.38±6.01 msec vs. mean duration 87.41±13.09 msec with mean duration narrowing 1.24±1.92 msec in group B, p<0.001),the 60th minutes QRS duration narrowing showed marked significant difference for good reperfusion group A (mean duration 73.77±6.7 msec with mean duration narrowing 13.69±9.76 msec vs. mean duration 85.59±12.35 msec with mean duration narrowing 3.06±3.15 msec in group B, p=<0.001).

This study demonstrated the significance of 60th minutes mean QRS duration narrowing after primary PCI as dependable marker of tissue reperfusion after primary PCI.
