Background: Endothelial barrier function is important. Current drug eluting stents (DESs) using biodegradable polymer technologies are now available. However, little is known about endotheliual barrier function after biodegradable versus durable polymer DES.
Methods: Ultimaster® (UM, biodegradable polymer based abluminal coating, sirolimus-eluting stent), Xience®(XI, durable polymer based conformal coating, everolimus-eluting stent) and Kaname®(KA, bare metal stent) were compared. In rabbits model (n=45), stents were implanted to denudated iliac arteries. At 28, 45 and 120 days, stented arteries were harvested. Eight animals at 28 days were used for light microscopic assessment. In the rest of the animals (n=37), samples were stained with immunofluoresenct staining against endothelial barrier protein vascular endothelial cadherin (VE-cad) and observed by confocal microscopy followed by observation using scanning electron microscopy (SEM). In cell culture model, human umbilical vascular endothelium cells were cultured in stented silicone tube with 24 hours flow condition (15ml/minute) following 24 hours static condition and cell proliferation was assessed using bromodeoxyuridine (BrdU).
Results: Light microscopic observation was similar between UM and XI in terms of injury, inflammation struts with giant cells and fibrin. Strut coverage by SEM was superior in UM versus XI but was as much as in KA(82% in UM, 67% in XI, 100% in KA, P<0.01, at 28 days). Using confocal microscopy, KA showed nearly complete endothelial maturation by VE-cad at 28 days while both of DESs showed delayed maturation although UM showed numerically better maturation than XI (39% in UM, 22% in XI, 95% in KA, P<0.01, at 28 days). VE-cad negative endothelium showed greater numbers of surface monocytes by SEM compared with VE-cad positive area although no significant difference was observed between stents. Endothelium cell proliferation was grater in UM than in XI but it was not as many as KA (Number of BrdU positive cells/mm2, 4 in UM, 0 in XI, 38 in KA, P<0.05).
Conclusions: UM showed superior strut coverage with greater endothelial maturation versus XI associated with less endothelial cell anti-proliferative effect.
