Background: Patients with diabetes and multivessel coronary artery disease (MVCAD) treated with multivessel percutaneous coronary intervention (MVPCI) are associated with higher hospital costs. The objective of our study was to assess the trends in the cost of MVPCI in diabetics and compare that to MVPCI in all patients including non-diabetics.
Methods: Data was retrieved from nationwide inpatient sample from the year 2006 to 2011. This was a 20% stratified probability sample of discharges from all the community hospitals participating in Healthcare Cost and Utilization Project. The International Classification of Diseases, Ninth Revision, Clinical Modification (ICD-9-CM) codes were used to identify diabetic and non-diabetic patients who underwent MVPCI with stent placement in >2 vessels. Trends for inflation adjusted mean hospital charges were computed for 24 quarters from the year 2006 to 2011.
Results: A total of 41,325 diabetic patients had ≥2 vessels percutaneous coronary interventions (PCI) and 2,406 diabetic patients had ≥ 3 vessels PCI. The mean age was 65 years (standard deviation ± 12). There were 39% females and 61% males (p<0.001). The mean charges associated with ≥2 vessels PCI group steadily increased from $69,671 to $95,632 in diabetics in comparison to $68,161 to $91,938 in all patients across the period. The mean charges for >3 vessels PCI group increased from $87,478 to $108,919 in diabetics in comparison to $82,045 to $105,365 in all patients across the period. The positive linear trend of mean charges for each case was statistically significant (p<0.001).
Conclusions: There was a steady linear increase in the total charges for MVPCI procedures performed in diabetics and all patients from the year 2006 to 2011. The results of this study can be used to assess health care delivery cost associated with the use of MVPCI in all patients and to inform policy to reduce cost.
