Background: The associations between CAC burden and 10-year ASCVD risk estimates using the 2013 ACC/AHA Pooled Cohort Equations (PCE) across ethnic groups are not well described.
Methods: We included non-statin users (age ≥45 and <80 years) from the MASALA study of South Asians (SA) and the MESA study of Non-Hispanic Whites (NHW), African Americans (AA), Hispanics (HP), and Chinese Americans (CA). 10-year risk was estimated using the PCE. The proportions of participants with CAC=0 and CAC>100 were compared across levels of estimated ASCVD risk (<5%-low; ≥7.5%-high), by ethnic group. We used logistic regression models to estimate the associations between ethnicity and CAC burden among risk strata.
Results: Baseline characteristics are shown in Table1a. AA had the highest median estimated risk (10%); SA had the lowest (4%). Among low risk persons, the highest prevalence of CAC>100 was observed in NHW, CA, and SA (p<0.001). Among those at high risk, NHW and SA had the lowest prevalence of CAC=0 (p<0.001). In multivariable analyses (Table1b), among low risk persons, AA and HP ethnicities had significantly lower prevalence of CAC>100 compared to NHW; for those at high risk, AA and HP ethnicities had a higher presence of CAC=0 compared to NHW. CAC prevalence was similar between SA and NHW for low and high risk outcomes (p<0.001).
Conclusions: Discordance between CAC burden and estimated risk varied across ethnic groups with the most discordance among AA and HP. CAC levels and ASCVD risk was most concordant among NHW and SA.
