Background: Vascular remodeling associated with increased extracellular matrix (ECM) may precede hypertension (HTN). Procollagen Type III NTerminal Propeptide (PIIINP) and Collagen Type I CarboxyTerminal Telopeptide (ICTP) reflect collagen synthesis and degradation and are important in ECM remodeling. We hypothesize that PIIINP and ICTP predict incident HTN in normotensives without cardiovascular disease (CVD).
Methods: We included 1202 MESA subjects (mean age 58.3±12.5 years, 47% male) and free of overt CVD, having a systolic blood pressure (SBP) <130 mmHg and diastolic (DBP) <80 mmHg, not using antihypertensive medication (Rx), with plasma PIIINP and ICTP measurements at baseline. New onset HTN (35%) was defined as SBP ≥ 140 mmHg and/or DBP ≥ 90 mmHg, or antihypertensive Rx use during the 2nd, 3rd, 4th and 5th followup visit (median 10 years). The relative risk (RR) per standard deviation (SD) of the log of PIIINP and ICTP and incident HTN was calculated in model 1 (adjusted for age, race and sex) and model 2 (model 1 + baseline eGFR, height BMI, heart rate, SBP, DBP, smoking, cholesterol, HDLchol, triglycerides, cholesterol lowering Rx, diabetes and CRP).
Results: Mean±SD PIIINP was 5.38±1.94 μg/L and ICTP was 3.18±1.40 μg/L. Baseline SBP was 110.3±13.8 mmHg and DBP 67.0±9.6 mmHg. Higher values of PIIINP and ICTP preceded incident HTN (Table 1).
Conclusions: ECM remodeling, probably vascular related, preceded the development of hypertension.
