Background: Associations between extracellular matrix (ECM) metabolism and circulating microRNAs with right ventricle (RV) in dilated cardiomyopathy (DCM) are unknown.
Methods: We studied 70 DCM patients (pts) (48 ± 12.1 years, EF 24.4 ± 7.4%). RV abnormality was diagnosed if RV was dilated: basal RV (RVd1) > 41 mm and/or mid-cavity RV dimension (RVd2) > 35 mm and/or RV systolic function was impaired: TAPSE < 17 mm and/or RV tissue lateral systolic velocity (RV-S’) < 9.5 cm/s. Markers of collagen type I and III synthesis – procollagen type I and III carboxy- and amino-terminal peptides (PICP, PIIICP, PINP, and PIIINP), collagen 1 (col-1), fibrosis controlling factors – tumor growth factor (TGF1-β), connective tissue growth factor (CTGF), and osteopontin (OPN), and degradation enzymes – matrix metalloproteinases (MMP-2, MMP-9) and their tissue inhibitor (TIMP-1) were measured in serum. Plasma expressions of miR-21, miR-26, miR-29, miR-30 and miR-133a were measured with quantitative PCR. Three logistic regression models were constructed: unadjusted model (1st model), model with the adjustments to the age, sex, BMI and disease duration (2nd model), and 3rd model with the adjustments to age, sex, BMI, disease duration, EF, right atrial area, ECM fibrosis, pulmonary hypertension, and NT-proBNP to study associations between ECM markers and microRNAs with RV morphological and functional parameters.
Results: RV abnormality was found in 55 (78.6%) pts. Not a single marker of ECM fibrosis was independently associated with RV morphological and functional parameters. Out of five microRNAs studied, only miR-30 was associated with RV impairment (OR 0.41; p<0.02) in the 2nd model.
Conclusions: RV dilatation and/or systolic dysfunction was common and observed in more than three-quarter of DCM pts. Serum markers of ECM metabolism are not useful in predicting RV abnormalities. Mir-30 was independently associated with RV impairment in the 2nd model, however, this association was no longer observed in the most comprehensive model that was adjusted for more powerful RV-associated variables. The prognostic value of studied microRNAs on RV parameters is limited in DCM.
