To explore the value of two-dimensional speckle tracking echocardiography in quantitative analysis of left ventricular myocardial longitudinal layer-specific strain.
Totally 82 subjects with no significant coronary artery stenosis (<50%) confirmed by coronary angiography were involved in this study. All subjects were divided into two groups according to their sex: male group (n=45), female group (n=37). And then they were divided into three groups according to their age: groupI(n=16, aged from 37-50 years old), group II(n=45, aged from 51-60 years old), group III(n=21, aged from 61-70 years old). The conventional echocardiography was performed in all subjects by application of GE Vivid E9．A tracing analysis was conducted on the off-line EchoPAC workstation after storing the two-dimensional dynamic images．The peak systolic longitudinal strain (LPS) of whole stratification and full-thickness were obtained from inner，medial and outer myocardial (subendocardial, mid-myocardial and epicardial) layers of 17 segments in the left ventricular walls.
The absolute value of global longitudinal peak systolic strain (GLPS) increased layer by layer in epicardial, mid-myocardial and subendocardial layer (P< 0.001). In each layer of myocardial, the absolute value of longitudinal peak systolic strain increased segment by segment in basal, papillary muscle and apical level (P<0.001). In case of basal segment, the absolute values of longitudinal peak systolic strain increased layer by layer in epicardial, mid-myocardial and subendocardial layer (P<0.05). In the papillary muscle and apical level, the absolute values of longitudinal peak systolic strain increased layer by layer in epicardial, mid-myocardial and subendocardial layer (P<0.001). Absolute values of GLPS of the three-layer myocardial in female group were higher than that in male group (P<0.001). No significant difference was found between the different age groups in GLPS.
Two-dimensional speckle tracking imaging can assess the left ventricular layer-specific strain. The strain value can reflect the systolic capacity of myocardial in different segments. The absolute values of global longitudinal peak systolic strains of the three-layer myocardial in female group were significantly higher than those in male group.
